Physical Examination

A student enrolling in a District school for the first time is encouraged to have a physical examination by a licensed medical provider.

Vision and Dental Screenings

The District shall work with parents and guardians of students to meet the requirements for vision and dental screening of students as required by the Iowa Department of Public Health.

Immunization Certificate

In accordance with state law, all students enrolled or attempting to enroll in a public elementary or secondary school in Iowa including those who are provided competent private instruction MUST submit proof of immunizations to the school’s admitting official with a valid Iowa Department of Public Health certificate or provisional certificate of immunization unless a valid certificate of religious or medical exemption has been submitted. Certificates of exemption must meet the requirements under state law. Immunization requirements and exemptions from requirements are specified in the Iowa Department of Public Health regulations and can be found on the Department’s website. Exemptions do not apply in times of emergency or epidemic as determined by the State Board of Health and as declared by the Director of Public Health.
